Job Description
Schedule: Full-TimeDay - M-F - 8 am-4:30 pm rotating weekends

How you'll contribute
A Social Worker who excels in this role:
- Assess and plan interventions to help patients cope with social, emotional, economic, and environmental problems.
- Interview and assess patients and/or patient's family, caregivers, and/or legal representative.
- Participate in case reviews to evaluate case management progress.
- Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
- Assist with discharge planning and process.
- Identify appropriate resource, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
