Job Details

Assess and plan interventions to help patients cope with social, emotional, economic, and environmental problems. Interview and assess patients and/or patients family, caregivers, and/or legal representative. Participate in case reviews to evaluate case management progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patients treatment plan. Assist with discharge planning and process. Identify appropriate resource, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Why join us.
